Request a Statement of Self Retention
The general policy of the State of New York is self-assumption of risk where no insurance is ordinarily obtained to protect state-owned property or for third-party claims arising from injuries to the public at state-owned facilities, sometimes known as “Self Insurance” or “Self Retention.” You may request a Statement of Self Retention by emailing [email protected].
Request a Certificate of Insurance for General Liability
Request coverage for an event such as the use of space for a meeting or training, display booths, classroom use, etc. The limit of liability is $1,000,000/$2,000,000 aggregate. If you are requesting coverage for an event that does not fall within the policy’s current schedule of coverage, you may be charged a premium to add the requested coverage. Any certificates that result in additional premium will be forwarded to the requesting agency for approval prior to being issued.

Request a Fine Arts Certificate of Insurance
Request coverage for objects that are on loan to a New York State agency. There is no coverage for state-owned artwork. If your agency does not currently have coverage at the requested location, OGS will contact you for additional information. If you need to increase your current limit, please indicate the new limit on your certificate request form.

Request Other Types of Certificates
To request other types of certificates, such as auto, inland marine, builders risk or property, send an email to [email protected] and the appropriate form will be sent to you. The OGS Bureau of Risk & Insurance Management staff will review your request and generate a Certificate of Insurance as appropriate.
Request New Insurance Coverage
The general policy of the State of New York is self-assumption of risk where no insurance is ordinarily obtained to protect state-owned property or for third-party claims arising from injuries to the public at state-owned facilities, sometimes known as “Self Insurance” or “Self Retention.” In order to request a new policy, there must be a contractual obligation or special circumstance.
To request, send an email to [email protected] with the following information:
- Attach the contract document highlighting the insurance requirements
- Requesting Agency
- Agency Contact Names (please provide at least 2)
- Agency Contact Email Addresses
- Agency Contact Phone Numbers
- Description of the type of coverage needed
OGS will review your request and follow-up for additional information. Once a premium quote is received, it will be sent to the agency for approval prior to coverage being bound.
