Report and Replace a Lost or Stolen ID Card
- Immediately report a lost or stolen ID card to OGS at 518-474-6292 or by email at [email protected].
- Notify your agency's ID card representative to order a replacement. If you do not know who your representative is, contact OGS. Replacements typically take 7-14 business days from the date the application is received by OGS.
- A $13.00 replacement fee is charged.
- Employees must enter the building as a visitor until the replacement card is received.
Replace a Damaged or Non-Working ID Card
- Notify your agency's ID card representative to order a replacement. If you do not know who your representative is, contact OGS at 518-474-6292 or by email at [email protected]. Replacements typically take 7-14 business days.
- A $13.00 replacement fee is charged.
- Employees must enter the building as a visitor until the replacement card is received.
- OGS will not levy a replacement charge for cards that:
- No longer work, with no signs of damage
- For a change of name or updated picture on your driver's license.
- When a card becomes too faded to read.

Rules of Use
All OGS issued ID cards are the property of New York State. Their use is subject to restrictions outlined in Title 9, Section 300-3.1(t) of the New York Code of Rules and Regulations as follows:
- No person shall convey or loan their New York State employee/vendor identification card to another person for the purpose of allowing said person(s) to gain unauthorized entry onto State property.
- Any person found to be in possession of a New York State employee/vendor identification card who is unauthorized to carry such card shall be subject to prosecution under the provisions of article 165 of the Penal Law of the State of New York, or any successor provisions thereof or any other applicable provisions of the Penal Law.
- All lost or stolen New York State employee/vendor identification cards shall be immediately reported to the OGS Security ID Card Unit at 518-474-6292 to provide a report of lost or believed stolen ID Cards.
- Upon separation from State service or termination of contractual services, the New York State employee/vendor identification card shall be surrendered to the issuing agent. Failure to do so may result in criminal prosecution as set forth in paragraph (2) of this Section.
In addition, alteration, duplication or destruction of the cards is also prohibited. Cards shall not intentionally be used to allow access to facilities to any person in addition to the cardholder, even if accompanied by the cardholder. Security ID cards are non-transferable, and may not be given by the cardholder to any other person for any reason.
Violations
Violations of the rules of use may result in confiscation of the card and/or a suspension of some or all of the card’s access privileges.
- If the card is confiscated, it will be returned within two business days.
- First offense: Suspension of access privileges for violations may extend for a period of up to 14 days.
- Second offense: Suspension of access privileges for violations may extend for a period of up to 30 days.
- Third offense: A third violation may result in revocation of access permissions indefinitely.
The length of the confiscation/suspension period will depend upon the severity of the violation and whether the violator has any prior violations. Additionally, at the discretion of the employing agency, violations by state employees may also form the basis of disciplinary actions in accordance with applicable collective bargaining agreements. Finally, criminal charges may be pursued, as provided for above in the OGS regulations, or if the elements of any crime such as fraud exist in any other situation.
Frequently Asked Questions
Are ID cards also used for parking access?
Yes. ID cards are used for both building and parking access. If you get a new or replacement ID card, contact OGS Parking Management to request your parking access be added to the card.
Can I access any OGS-owned/managed facility with my ID card?
No. New York State employees, vendors, and consultants are given building access permissions only to where they work or conduct official business.
What if I forget my card?
Employees who forget their card must present another form of ID and are admitted to the building as a visitor.
Can I get a duplicate card to leave in my vehicle for parking access?
No. Only one card is issued per employee.
