OGS Procurement Services Centralized Contract Eligibility Application

If you believe your organization may be eligible for the New York State Contract Extension Program and you are interested in participating in the program, please submit the application form(s) appropriate for your type of organization (which can be found below in the “Forms” section of this page). All forms must be completed and submitted by an authorized representative of your organization.

Note: Submitting the application form(s) does not guarantee an organization’s acceptance into the program. Once your application and information have been reviewed, you will be notified by OGS Procurement Customer Services regarding whether you have been accepted or your application has been denied.

 

Volunteer Fire and Ambulance Companies:

Not-for-profit volunteer fire and ambulance companies interested in participating in the New York State Contract Extension Program must submit the required application and certification of qualification forms. Please refer to New York State General Municipal Law Sections 100 and 104, as amended, which allow certain New York voluntary fire companies and ambulance services to make purchases through the Office of General Services, along with purchases from other public contracts, including certain federal contracts. Applicants should note that they will need to provide their NYS Department of Health or NYS Office of Fire Prevention & Control departmental ID numbers on the certification form.

 

Charitable Organizations:

Chapter 741 of the Laws of 1985, last amended by Chapter 55 of the Laws of 2012, allows certain charitable organizations to make purchases from commodity contracts awarded by the Procurement Services division of the New York State Office of General Services. Qualifying entities interested in participating in this program should download, complete, and submit the application package below.
